Healthcare Services Division Contract Monitoring Bureau

The Healthcare Services Division Contract Monitoring Bureau (MSCMB) was established to follow the medical care and treatment of their patient population after the privatization of health services began on July 1, 2012. 

The MSCMB partners with the State of Arizona’s contracted health services providers to ensure that quality health care initiatives are consistently maintained. The Bureau also ensures that the ADCRR’s health services providers comply with all aspects of their contract. This provision of care includes medical, mental health, and dental services. These services are provided in accordance with all applicable laws, rules, and regulations; the American Correctional Association; the National Commission on Correctional Health Care; and ADCRR policies and procedures.

Effective October 1, 2022, Arizona Department of Corrections, Rehabilitation & Reentry contracted with NaphCare to provide full service medical, mental health, and dental care services to patient populations housed at the following Arizona State Prison Complex's (ASPC):

  • Perryville
  • Phoenix
  • Safford
  • Tucson
  • Winslow
  • Yuma

If you have questions regarding medical, mental health or dental care services concerning an incarcerated individual who is a family member or a friend in one of the facilities listed above, please call (800) 341-7024 or send an email to [email protected].

 

Note: Please note that inquiries not related to health care will not be processed.

Due to the confidentiality of medical records, a release of information from the inmate may be required in order to provide specific details of any care, condition, or treatment plans.

 

Medical Records

To request copies of medical records for inmates incarcerated at one of the Arizona Department of Corrections, Rehabilitation & Reentry facilities listed above, you may call (833) 378-0555 or email your request to [email protected].

You may use a properly executed ADCRR authorization form or your own HIPAA compliant form.

 

Note: The ADCRR form will only allow release of records up through the date it is signed, regardless of the date range entered on the form.

Note: There is a fee of $0.50 per page for paper records. Records that are both maintained and produced electronically will not be assessed a fee.
